  1. Patient must notify of any error w/in 60 days after a statement is mailed.
  2. Hosp must reply to the complaint w/in 30 days of receiving it.
  3.  Error must be corrected or the accuracy explained w/in 2 billing cycles, or 90 days maximum.
  4. If any stipulations are violated, patient rights are violated and collection may be forfeited.
