anti-Ro (SS-A): associated with neonatal lupus and fetal heart block

[mother's Ab is carried across the placenta to cause either fetal heart block by damaging fetal heart tissue or causing rash and cytopenia in newborn; goes away once mom's antibodies leave baby's system (months)]

RF

85% sensitive, but not specific; can be + for Sjogren’s (75%), Hep C (50%) and in normal pts (5%)

Anti-CCP

[cyclic citrullinated peptides] (60% sensitive, 95% specific to RA)

o   Anti-CCP may play a pathogenic role in RA

o   Citrullinated proteins (fibrinogen) are present in the cytoplasm of cells in rheumatoid pannus, and also in the interstitial areas

§  are thought to provide persistence antigenic stimulation that drives the immune response in RA

Polyarticular JIA

Definition

RF+

[2-10% of JIA; teens, increased HLA-DR4, resembles adult RA; rare remission & poor fxnl outcome]

RF-

[10-30% JIA; symmetric small jt dz; better fxnl]

Term
Pauci/oligoarticular JIA
Definition

(+) ANA

two types:

persistent and extended
