Cards

Term
Amantidine and Rimantidine
Definition
  • Anti-influenza drug
  • inhibit viral protein translocator which acidifies the core of the viral particle
  • prevents unpackaging
  • oral, 20hr, renal elimination
  • Used with L-dopa for parkinsons; most flu strains resistant
Term
Zanamivir and Oseltamivir
Definition
  • Neuraminidase inhibitor
  • Prevent viral release, limit spread, reduce epithelial cell apoptosis
  • Used for Influenza A & B, some H5N1 resistant
  • prophylaxis 70-85% effective; reduces symptoms when given w/in 30 hr of infection
  • nontoxic; Nausea, vomiting, headache
Term
Idoxuridine
Definition
  • Thymidine analog
  • Phosphorylated by TKs and incorporated
  • Changes pKa of H in base pairing->mutation, alters DNA conformation, DNA breakage
  • Mutagenic, carcinogenic, teratogenic
  • Used topically only: HSV, keratitis
Term
Acyclovir
Definition
  • Mech:Acyclo-dGTP compet inhib dGTP @ DNAP, Chain terminator when incorporated
  • Oral or iv, 3 hr, urinary excretion
  • Wide distribution, including CNS
  • Low toxicity, no bone marrow suppresion
  • Use: HSV1,2, EBV, not for CMV
Term
Ganciclovir
Definition
  • phosphorylated by cellular and HSV TKs
  • More actively phosphorylated in infected cells
  • Comp inhib dGTP incorp by DNAP, term chain
  • Not as specific as acyclovir
  • Valgancyclobir=prodrug, increases absorption
  • IVm 3-4hr renal elimination
  • bone marrow depression, nephrotoxic, GI
  • Use: CMV, retinitis, all HSV(2nd line)
Term
Entecavir
Definition
  • Guanosine nucleoside analog
  • Inhibits HBV: DNAP base pairing, RT on the (-) strand, syn of (+) strand of HBV DBA
  • Renal elim
  • Headache, fatigue, N,V, vertigo, cancer in animals, lactic acidosis, hepatomegally
  • Use: Hep B, severe excacerbation if discontinue
Term
Adefovir
Definition
  • phosphorylated and a substrate for hepadnavirus DNAP>host DNAP, incorp into viral DNA acting as chain terminator
  • Renal elim
  • Nephrotoxic, lactic acidosis, hepatic steatosis if used with NRTIs for HIV
  • Use: Hep B
Term

Ribavirin

Definition
  • Reduces GTP synthesis and inhibits viral RNAP
  • Inhibits mRNA guanyltransferases, labilizes viral mRNA by reducing 5' cap formation
  • Oral, IV, aerosol, t1/2=2 hr, hepatic metab, urine elim
  • bone marrow depression, GI, CNS, teratogenic, mutagenic, carcinogenic
  • Use: Hantaan RSV bronchiolitis, pneumonia, +interferon alpha for Hep C
Term
Penciclovir
Definition
  • Acyclovir analog
  • Use: HSV1, 2, EBV
Term
Famciclovir
Definition
  • Acyclovir prodrug
